Changes to Bankruptcy Forms 2006

Three bankruptcy forms and the instructions for a fourth form will be revised effective April 9, 2006, as a result of the enactment of the Deficit Reduction Act of 2005, Pub. L. No. 109-171, which changed the filing fees for chapter 7 and chapter 13 cases. The following bankruptcy forms and instructions will be revised:

Official Form 3B, Application for Waiver of Chapter 7 Filing Fee

Instructions for Official Form 1, Voluntary Petition

Director's Procedural Form 200, Required Lists, Schedules, Statements and Fees

Director's Procedural Form 201, Notice to Individual Consumer Debtor

Copies of these forms are posted on the United States courts
